Required Documents/Process
- Request letter
- Completed application form with Grama Niladhari recommendation
- Documents related to proof of income
Ex:
- If the income is from tea cultivation, a certified copy of the land and leaf bills for the 03 months closest to the date of obtaining the income certificate
- Certificate of business registration if the source of income is from a business
- Salary details if the source of income is from employment

Issuing Value Certificates
Required Documents/Process
- Request letter
- Completed application form with Grama Niladhari recommendation.
- Certified copy of the deed of immovable property to which the certificate of value is applied.
- Sheet of the relevant deed one month before the date of applying for the Certificate of Value.
- (Feez = Annual Income * 1.27%)
