Special Notice
All applicants who come to obtain the essential service permits should submit the following documents.
1. Letter of request
2. Application submitted by us
3. Recommendation obtained from the Grama Niladhari of the division to be a suitable person to obtain a license and not to quarantine
4. Photocopy of the vaccination card confirming that the vaccine was obtained
5. Copy of the business registration certificate confirming that you are doing business
6. Old Essential Service License that you have obtained (if any)
