1) How to get copies of birth, marriage or death certificate?
- Charge 100 rupees, if you know the number and date of the birth or marriage or death certificate,
- If you do not know the number and date of the birth or marriage or death certificate, you can also obtain a certificate by paying Rs. 200 to the Divisional Secretariat.
- Relevant certificates can be obtained from this office only in case of birth, marriage or death in the Ratnapura District (1960-2010).
2) How to register a birth?
- To register a normal birth, the birth should be registered by meeting the Registrar of Births Registration of the relevant division within three months of the birth.
- Contact the Additional District Registrar and obtain relevant information for registration of births after three months from the birth.
3) How to amend the birth certificate?
Take the relevant documents and meet with the Additional District Registrar.

1) How to make Widows' and Orphans' Pension after Civil Retireer dies ?
-First obtain the death certificate of the retired person and terminate the pension.
-If there is more money, write to the bank to get it.
-Then instruct the relevant person to bring the following documents.
- Completed PD 04 Source Document certified by the Grama Niladhari
- Original death certificate of the retired person
- Original birth certificate of the widow
- Original marriage certificate
- A photocopy of the widow's bank book
- A photocopy of the widow's identity card
- A Photocopy of Widow Orphan Card or Widow Orphan Number
- Passport size color photograph certified by the Grama Niladhari
- An affidavit if there is a change in the widow's name
- All photocopies of the above documents should be certified by the Grama Niladhari.
- Required original birth certificates, if there are unmarried/disabled children under 26 years old.
Then after giving the relevant information, take the file and take necessary action to make the Widows' and Orphans' Pension.

1. Application and request letter for registration
2. True copies of the rules of the voluntary social service organization (Code of Conduct)
3. Report of the general meeting elected by the officers of the organization and the attendance register
4. Membership Register
5. Details of the officers of the society
6. Authentic copy of the latest statement of accounts including balance sheets certified by a recognized auditor / cooperative inspector
7. Proposed program to be implemented for the coming year
8. Map showing the covered area
9. 03 consecutive meeting reports and attendance registers
10. A copy of the bank book
